Tubo‐ovarian abscess (TOA) is a recognised and serious complication of untreated pelvic inflammatory disease (PID). It most commonly affects women of reproductive age and nearly 60% of women with TOA are nulliparous. 1 TOA is defined as an inflammatory mass involving the tube and/or ovary characterised by the presence of pus.

Tubo-ovarian abscesses are one of the late complications of pelvic inflammatory disease and can be life-threatening if the abscess ruptures and results in sepsis. It consists of an encapsulated or confined 'pocket of pus' with defined boundaries that forms during an infection of a fallopian tube and ovary.

Four cases of unilateral tubo-ovarian abscess in patients using the IUD are presented. Pelvic inflammatory disease, also known as pelvic inflammatory disorder (PID), is an infection of the upper part of the female reproductive system, namely the uterus, fallopian tubes, and ovaries, and inside of the pelvis.

We describe a case that presented seven years after post partum tubal sterilization, showing both acute and chronic components. The reasons for such immunosuppression can be exacerbation of extragenital and genital pathology, severe catarrhal diseases, frequent stresses, increased physical exertion, prolonged use of corticosteroids, cytotoxic drugs, acetylsalicylic acid derivatives, etc. These causes are most often noted in the history of tubo-ovarian abscess.
